의사들이여, 10년 후를 대비하라

 

기업의 사업포트폴리오를 성장률과 시장점유율에 따라 스타(star), 캐시카우(cash cow), 물음표(question mark), 개(dog) 등 4가지로 구분하는 ‘BCG매트릭스’ 가 있다.

 

삼성전자를 예로 들자. 성장률도 높고 시장점유율도 증가하고 있는 핸드폰과 평면TV는 스타(star)다. 반도체는 성장률이 주춤하고 수익률도 더 이상 높아지지 않는다. 이미 투자를 해놓았기에 경기가 좋아지면 수익은 크게 나고 현금유입이 가능하다. 따라서 캐시카우(cash cow) 다. 백색가전은 중국업체의 추격세가 두드러진다. 국내는 임금이 비싸서 상대적으로 수익률도 매우 떨어진다. 개(Dog)다. 태양전지, 인공지능 등은 앞으로 성장가능성이 높다. 하지만 불확실성 역시 매우 크다. 따라서 물음표(Question mark) 다. 도표로 보면 다음과 같다.

그렇다면 삼성전자는 앞으로 어떻게 해야 할까? 스타에 해당되는 평면 TV와 핸드폰에는 지속적으로 투자를 해야 할 것이다. 하지만 스타도 어느 순간 성장률이 정체되면 캐시카우가 된다. 캐시카우인 반도체에는 투자를 하되 투자의 목적은 수익성을 통한 현금 확보에 두어야 할 것이다. 캐시카우에서 확보한 현금으로 스타에 지속적으로 투자를 해야 한다. 그러나 언젠가 스타도 캐시카우가 된다는 점을 고려할 때, 물음표에 해당되는 태양전지, 인공지능의 상용화에 투자해서 물음표가 향후 스타가 될 수 있도록 해야 한다. 개(Dog)에 해당되는 백색가전에 대해서는 더 이상 투자를 하지 않고 수익을 통해 현금만 확보하다가 기업가치나 브랜드가 어느 정도 남아 있는 시점에서 중국이나 인도의 가전업체에 매각을 검토해야 한다.

 

그렇다면 병원에도 이 BCG 매트릭스를 도입할 수 있을까? 대학종합병원을 예로 들어보자. 요사이 무더기로 병상이 늘어나고 있는 암병동은 현재 대학병원의 스타다. 암환자는 앞으로도 증가할 것이고, 비급여항목도 많다. 암치료는 전세계적으로 가장 투자가 활발히 이루어지고 있고, 따라서 고가의 비급여 의료기술이 지속적으로 상용화되기 때문에 의료보험수가로부터도 상대적으로 자유롭다.

 

캐시카우는 건진센터와 장례식장이다. 현재 대학병원은 암센터를 제외한 진료분야에서 나온 적자를 건진과 장례식장에서 메우는 사업구조를 가지고 있다. 그러나 건진센터와 장례식장의 성장성은 미지수다. 정부가 암검진 사업을 지속적으로 추진하고, 의료 소비자단체가 활성화되면 현재의 고가 전략에 기초한 호화 건진센터는 타격을 입게 된다. 환자들도 인터넷등을 통해서 더욱 정보를 많이 얻게 된다. 사실 건강검진에서 꼭 필요한 것은 위암, 대장암, 자궁경부암, 유방암, 전립선암과 같은 발병률이 높은 암에 대한 검진이다. 임상증상이 있으면 병원에서 보험적용을 받을 수 있는 검사를 비급여로 검진센터에서 진찰 받을 필요가 없다는 것을 환자들도 점점 더 알게 될 것이다. 장례식장도 각 대학병원이 돌아가면서 시설을 확충하고 있다. 한 때 삼성병원 장례식장이 가장 좋았지만, 지금은 아산병원 장례식장이 가장 좋다. 하지만 앞으로 모든 대학병원이 돌아가면서 리노베이션을 하거나 신축을 할 것이다. 그리고 장례산업의 수익사슬(Value Chain) 을 바꾸고 있는 것이 상조회사다. 현재는 상조회사가 서민층을 대상으로 기반을 확충하고 있지만, 도시중산층에 대한 상조사의 영향력이 커지고 있다. 상조사의 협상력이 강해지면, 그것은 직간접적으로 장례식장의 수익률 저하로 이어질 수도 있다. 따라서 건진센터와 장례식장은 캐시카우이지만 성장의 폭은 제한적일 것이다.

 

대학종합병원 응급실, 중환자실, 외래, 그 외 입원 진료 부분은 수익률이 극히 낮거나 아니면 손해를 보고 있다. 따라서 그 수익률, 성장률만 놓고 보면 개(Dog)에 해당이 된다. 하지만 스타와 캐시카우에 해당되는 사업부분을 이용할 고객집단을 확보하기 위해서는 개(Dog) 에 해당되는 진료사업부분이 꼭 필요하다. 일반진료에서 확보된 고객군이 있어야 장례식장, 건진센터가 캐시카우로 작용한다.

 

그렇다면 물음표에 해당되는 것은 어떤 부분이 있을까? 안면이식, 양팔이식 등과 관련된 이식술은 앞으로 성공만 한다면 스타가 될 수 있다. 1990년대 간이식, 심장이식이 서울아산병원의 스타였듯이 말이다. 혈액검사를 통한 조기 암건진도 스타가 될 수 있다. 지금도 내시경, 팝 스미어 등의 검사에 대해서 불편함을 호소하면서 기피하는 환자들이 적지 않다. 수면내시경이 도입되고 불편함이 많이 감소했지만 내시경 하면 몸에 뭔가 들어온다는 것 자체로 싫어하는 환자도 있다. 하지만 향후 혈액을 통한 암검사의 특이도와 민감도가 향상된다면, 암검사의 문턱을 낮추어 줄 수 있다. 만약에 혈액 검사를 했는데 대장암에 걸릴 확률이 40%로 나왔다면, 그 수치를 보고서는 제아무리 내시경 검사를 두려워하던 이들도 하게 될 것이다. 조기암검진이 발달하게 되면 한편 현재의 스타인 암병동의 수익성은 급격히 저하될 것이다. 조기에 암이 발견되면 간단한 수술로 치료를 받게 되고, 환자들이 고가의 항암치료나 침습성 암치료를 받는 것이 줄어들기 때문이다. 지금 우후죽순으로 늘어나는 암병동이 10년 후에는 개(Dog)에 해당되는 애물단지가 될 지는 아무도 모른다. 고령화가 되면서 의학적으로 확실한 증거가 있다면 노화를 방지하는 의료사업 역시 물음표에서 스타로 바뀔 수 있다.

 

지난 몇 년간 내부에서는 스타에서 캐시카우로 변하고 있다고 판단되는 사업분야에, 외부에서는 아직도 스타라고 생각하고 투자를 했다가 곤란을 겪은 예가 적지 않았다. MRI 기계를 설치한 방사선과 의원, 척추수술전문병원, 관절수술전문병원, 라식수술안과 등이 스타 단계를 기대하고 투자를 했다가, 막상 자신들이 투자비를 회수하는 시점에서는 캐시카우로 바뀐 상황에 처하고 당혹하는 것을 봤다. 정부의 예상치 않은 규제강화는 이러한 경향을 더욱 강화시켜 1~2년만에 스타를 개(Dog)의 상황으로 바꾸기도 하는데 그 좋은 예가 요양병원이다. 안 그래도 신규경쟁자 때문에 어려운 상황에서 정부는 수가를 제한하고, 노인요양원이라는 경쟁자군을 정책적으로 육성해서, 요양병원의 경영난을 야기했다.

 

그런 점을 봤을 때 동네환자들을 주로 보는 동네의원은 성장성은 크지 않지만 매출은 어느 정도 지속적으로 늘어나는 캐시카우로서 계속 존속할 가능성이 크다. 일단 고령화로 인해서 환자는 점점 늘어난다. 고혈압, 당뇨, 고지혈증, 관절염, 전립선비대증은 70세 이상 노인인구의 기본질환이다. 따라서 입지만 좋다면 서서히 환자가 쌓이게 되어 있다. 정부도 새로이 급격히 청구액이 늘어나는 질환군에 대해서는 수가인하, 기획실사 등을 하지만, 1차 의료에 대해서는 크게 터치하지 않는다. 따라서 동네의원의 경우 남들이 잘 되는 것 같다고 수익률이 높아 보이는 의료산업분야로 이동하기보다는 동네에서 보다 환자들이 접근하기 좋은 입지로 이동을 하고, 지속적으로 고객만족도에 신경을 쓰는 것이 캐시카우를 계속적으로 성장시키는 방법일 것이다. 10년 후를 대비한 가장 확실한 방법은 지금 내가 하고 있는 병원, 내가 진료하고 있는 환자를 더욱 사랑하고 아끼는 것이다. 그러면서 내 의료기관 내에서 할 수 있는 물음표에 대해서 지속적으로 관심을 가져야 한다.

 

10년 후 미래를 대처하기 위해서는 과거, 현재, 미래에 대한 시간 배분이 그 무엇보다 중요하다. 우리들 대부분은 우리에게 주어진 시간의 대부분을 현재를 위해서 사용한다. 그리고 나머지 시간은 이미 벌어진 과거의 사건에 대해서 돌이켜 생각하거나 분노하는데 사용한다. 정작 필요한 미래에 대해서는 시간을 쓰지 않는다. 지금부터라도 정기적금을 들듯이 미래를 위해서 시간을 써야 한다. 1990년대 자기 돈 내고 스터디 그룹에 들어가 임플란트 시술을 배운 치과의사들은 2000년대 큰 성장을 했다. 미래에 대한 투자가 현재의 수익으로 이어진 것이다. 향후 우리병원에서도 도입할 수 있는 신기술에 대한 좋은 배움의 기회가 있다면 오후 한나절 휴진을 하더라도 꼭 참가해라. 하루 30분 환자를 덜 보더라도 지속적으로 미래를 대비하는 시간을 가져라. 꾸준히 미래에 시간을 투자하는 것 그것이 이 격동의 의료산업에서 살아남는 방법이다.

신생혈관생성으로 눈이 먼 환자를 고친대요.

블록버스터를 노린다] <7> 한국노바티스 루센티스

황반변성은 눈 속에서 비정상적으로 생성된 신생혈관에 의해 황반이 손상돼 시력이 심하게 떨어지는 질환으로, 녹내장, 당뇨병성 망막증과 함께 3대 실명질환으로 꼽힌다.

한국노바티스의 루센티스(성분명 라니비주맙)은 연령관련 황반변성으로 손상된 환자의 시력을 회복시켜 주는 최초의 치료제다.

레이저 치료, 광역학 요법 등 기존 치료법은 시력 상실의 속도를 늦추거나 이미 손상된 시력을 유지하는 역할에 그쳤다.

이에 비해 루센티스는 신생혈관 형성에 중요한 역할을 하는 안구 내 혈관 내피세포 단백질에 선택적으로 결합해 새로운 혈관이 더 이상 자라지 못하게 하고 삼출물 누수를 차단함으로써 환자의 시력을 발병 이전 상태로 회복시키는 효과를 발휘한다.

루센티스의 시력 회복 효과는 2건(MARINA, ANCHOR)의 대규모 임상연구에서 확인됐다.

습성 황반변성 환자 716명을 대상으로 진행된 'MARINA' 연구 결과 루센티스 0.5mg으로 치료받은 환자의 95%가 치료 1년째 시력을 유지했고 40%는 시력이 개선된 것으로 나타났다.

423명을 대상으로 시행한 'ANCHOR' 시험에서는 기존 치료법인 광역학 요법을 사용한 대조군은 치료 1전째 시력을 상실한 반면 루센티스 투여군은 시력이 개선되는 효과를 보였다.

루센티스는 이런 획기적인 효과를 인정받아세계적 과학전문 잡지 '사이언스'가 선정한 2006년 10대 혁신적 연구성과로 꼽혔다.

서울대병원 안과 서울의대 유형곤 교수는 "루센티스는 유리체에 약물을 주사하는 유일한 약으로 효과가 뛰어나 환자들이 크게 만족스러워한다"며 "인구 고령화 현상에 따라 황반변성 환자가 크게 늘어날 것으로 보여 앞으로 루센티스의 역할을 증대될 것"이라고 말했다.

루센티스는 현재 미국, 스위스, 유럽연합, 인도, 호주 등 전세계 43개국에서 승인을 받았다. 국내에서는 2007년 7월 0.7mg을 허가받아 비급여로 출시됐지만 8월부터는 급여로 전환돼 좀 더 많은 환자들이 혜택을 받게 됐다.

Embryonic Stem Cell Substitute Passes Acid Test

Scientists have for the first time demonstrated that a possible alternative to controversial embryonic stem (ES) cells shares one of their key abilities. So-called induced pluripotent stem (iPS) cells can generate a live mouse in an experiment scientists say is the gold standard for pluripotency, the ability to become any cell type in the body. The feat, performed by two independent groups, suggests that iPS cells may be as powerful as ES cells, which many scientists hope will someday help cure a variety of diseases.

ES cells are prized by scientists for their flexibility. Taken from early embryos, they can in theory develop into all the tissues of the body. This talent makes them useful for studying--and perhaps someday treating--diseases. But because isolating ES cells involves destroying an embryo, they have proved ethically and politically controversial.

Many researchers believe iPS cells offer the best hope as an alternative. In 2006, scientists first demonstrated that by adding certain genes to adult mouse cells, they could reprogram them to look and act like ES cells. A year later, scientists achieved the same result using human cells. But it is still not clear how similar ES and iPS cells really are.

To grow ES cells into an animal, scientists inject them into an early embryo that carries twice the usual number of chromosomes. Such an embryo by itself can only become placental tissue. But if the injected ES cells are truly pluripotent, they can form a viable fetus. Several groups have tried the trick by substituting iPS cells for ES cells, but early attempts failed to produce any live-born mice.

Now, two groups in China report that their perseverance with the technically challenging experiment has paid off: They have produced apparently normal pups. A group led by Qi Zhou of the Chinese Academy of Sciences in Beijing and Fanyi Zeng of Shanghai Jiao Tong University used the method to test the pluripotency of six different iPS cell lines. In a paper published online today in Nature, they report that three of the lines yielded healthy mice at about the same rate as typical ES cell lines--about 3% of the time.

In a paper published today in Cell Stem Cell, Shaorong Gao of the National Institute of Biological Sciences in Beijing and colleagues describe a similar success rate with an iPS cell line that seemed to be particularly good at forming a variety of tissues in other tests. In nearly 200 tries, they produced several pups.

The results show that reprogramming can produce iPS cells--at least in mice--that are functionally equivalent to ES cells, says Konrad Hochedlinger, a stem cell biologist at Harvard University. But it also makes clear that all iPS cells are not equal. "It will be quite interesting now to identify the differences among different lines with different developmental potentials," he says. Although the test is an important proof of principle, it would be ethically impossible to do with human material. And in any case, it might not be relevant to the potential use of human iPS cells in therapies, he notes. "As long as the iPS cells make the functional cells you're interested in," he says, whether the cells are pluripotent may not be so important.

IPS로 쥐도 만들고 강아지도 만들었다는 이야기군요.

하지만 모든 IPS가 같은 능력을 가지는 것은 아니라고 합니다.

아래 두글을 정리해보면.. 대단한 결론이 나옵니다.

첫번째 실험 : 심근경색으로 심근세포 일부가 죽은 쥐의 완전 분화한 건강한 심장근육세포에 NRG1를 가했더니 분열이 일어나서 새로운 심근세포를 만들었다는 것입니다.

두번째 실험: 알츠하이머로 신경간 연결이 망가진 환자에게 신경줄기세포를 투입했더니
이 신경줄기세포가 신경으로 분화한 것은 일부이고 대부분은 다른 세포로 분화했으며,
BDNF라는 단백질이 이 줄기세포에서 분비되고 이것에 의해 망가진 신경세포들이
그 기능을 회복하는 쪽으로 형태변형과 재생이 일어났다는 것입니다.


이 의미는 무엇일까요?
대략적으로 말하자면 , 줄기세포는 기존의 손상된 부위를 치료하는 것이라고 말할 수 있겠지요.
더 정확히 말하면, 세포가 죽었으면 세포를 만들고, 단순히 연결이 손상된 것이라면 그것을 복구한다는 것이지요.  이것이 지금까지의 설명이었이었지요. 대단히 모호한 설명이지요.

더 정확히 말해볼까요?
성체줄기세포는 그 자체가 새로운 분화한 세포로 변화하기도 하지만,
거기서 나온 단백질들이 주위 완전 분화한 세포들을 자극하여 분열을 유도하기도 하고,
세포가 손상된 부분을 재생하기도 한다는 의미입니다.

지방세포의 경우에,

줄기세포지방이식의 원리는,
MSCs가 직접 새로운 지방세포가 되기도 하겠지만, 주위의 새로 이식한 지방세포들을 분열시키기도 할 것 같습니다.   PRP를 첨가하면,  재혈관화가 촉진되며, fibrogenesis가 촉진되어
지방세포가 더 오래살고, rejuvenation이 되는 것이겠지요.

혈소판이 중요한 게 아니고 그것에서 나오는 secretory proteins들이 역할을하여 창상치유를 촉진하는 것처럼,
MSCs도 그 자체역할보다 여기서 분비되는 secretory proteins들의 역할이 더 큰 것 같습니다.
 
 MSCs의 homing을 생각해보면,
지방추출시 준 injury site로 다시 MSCs 가 homing되므로,
수술시에 injury를 조금 주어야 정작 원하는 병소로 가는 MSCs의 수가 증가될 것이라는 것은 맞습니다.

그런데 수술시에 injury를 줄여도 완전히 피할 수는 없습니다.

그래서 어떤 선생님은 한번에 다 주지 않고 일부 보관해 두었다가 3개월 간격으로 주는 것입니다.

그럼, 이렇게 보관한 MSCs가 댕동 보관과정중에 다 터져 버려서 실제로 형태를 유지한 MSCs들이
얼마 없을 것이라는 의견이 있습니다.
일리 있는 의견입니다.

이렇게 터지고 손상된 MSCs들은 주입시에 효과가 없을까요?

상기 증례들을 보고 생각해 보변,
터진 MSCs들에서 나온 secretory proteins가 우리가 원하는 병소에 가서
이미 분화한 세포들을 자극하여 더 분열을 시키거나 손상된 세포들이 고쳐지도록 신호를 할 수 있다는 것을
짐작할 수 있습니다.
이때는 지방흡입부위의 상처는 이미 다 나았을테니까 더 원하는 부위로 갈 수 있겠지요?

이것이 냉동보관해서 수개월후 IV하는 것의 근거라고 생각이 듭니다.

그런데, 다르게 생각해보면, 이렇게 IV로 넣어주는 secretory protein들은 지속적으로 효과가 있는 것이

아니므로, 과연 그 효과의 정도가 얼마나 될지의심됩니다.

 

실제로, 국소로 성장인자만 주어도 줄기세포를 준것과 비슷한 효과가 있지만 줄기세포와 달리

지속적으로 성장인자가 해당부위를 자극하는 것이 아니고 일회성 자극으로 끝나기 때문에 효과의 정도가 떨어진다고 합니다.

 

이 경우에는 전신주사이기 때문에 더 주입한 성장인자의 농도가 떨어질 것이고 그렇다면 효과의 강도도 더 떨어질 것으로

짐작해 볼 수 있습니다.

 

결국 줄기세포의 보관을 잘 해야 겠습니다.

냉매제를 써서 천천히 얼리고 빨리 녹이는 방법을 써야 겠네요.






신경줄기세포로 알츠하이머 치매를 고친다.

신경줄기세포로 알츠하이머를 고친다는 이야기는 이미 많이 나온 이야기인데
이것을 동물실험 모델로 증명한 경우는 많지 않는 것 같습니다.

유전공학으로 조작하여 만든 치매쥐의 뇌에 신경줄기세포를 주입하였더니
쥐의 기억력이 호전되었다는 보고가 있네요.

과학자들이 호기심에 이 쥐의 뇌를 해부해 보았는데요.
주입한 신경줄기세포가 직접 뇌신경(neuron)으로 분화한 경우는 단지 6%밖에 되지 않았다는군요?
대부분은 지지세포인  astrocytes and oligodendrocytes로 분화를 했다네요.
그럼 어떻게 질병이 호전된 걸까요?

신경줄기세포가 새로운 뇌신경이 되는 것이 아니라,
기존의 뇌신경의 연결을 복구하고 손상을 치료한다고 합니다.

(synapse를 다시 연결해주고,  neurites를 새로 만들었다고 하네요)

그래서, 줄기세포가 천재로 만들지는 못하며, 비정상으로 변한 것을 정상적인 수준의 뇌로 만들 수는 있다는
이야기가 나오는 것입니다.

이것은 a protein called brain-derived neurotrophic factor, or BDNF라는 단백질이 신경줄기세포에서
분비함으로써 가능하다고 합니다.

결국, 줄기세포 자체가 아니라,
거기에서 분비되는 secretory proteins가 재생에 중요하다는 이야기지요.

이것은 줄기세포는 그 해당기관의 세포로 분화한다는 기존의 관념을 뒤엎는 결과입니다.

뇌신경에서만 이런 일이 일어나는 걸까요?

확실한 것은 secretory protein이 핵심이라는 것이고, 이것은 아마 다른 조직에서도 마찬가지 일겁니다.

과학자들은 다시 이 BDNF라는 것만을 주입해 봤는데요.
물론, 인지기능이 좋아지긴 했지만 신경줄기세포를 주입했을때보다는 못했다고 합니다.
이유는 신경줄기세포를 주입하면 자리를 잡고  오랫동안 지속적으로 BDNF를 분비하기 때문일 것이라고 합니다.


그렇다면,
기존의 세포들을 대체하는 것은 일부이고 대부분이  기존의 세포들을 고치는 개념이라면,
줄기세포가 그 부위에 인접해있기만 해도 되지 않을까요?
그 인접한 줄기세포들이 그 자리에 자리 잡고 계속해서 secretory proteins를 분비하면
인접한 손상된 세포들이 고쳐지지 않을까요?

아마도,줄기세포들에서 나온 seretory proteins는 줄기세포 스스로를 분화하게도 하고,
주위 세포들을 활성화하여 망가진 부분을 고치는 역할도 하나 봅니다.

즉, MSCs에서 나온 secretory proteins는 그 스스로를 자극하여, 지방이나 연골등이 되게 하는데,
이때, 주위 환경이 지방세포라면, 지방세포로 분화를 하고  주위 지방세포들이 손상받았다면
손상부위를 재생시켜서 오랫동안 살도록 하는 것 같습니다.
주위가 연골이라면 연골세포로 분화하고 , 주위 연골세포들도  고쳐서 원래대로 건강하게 되돌리겠지요.

줄기세포 없이도 심장근육을 재생할 수 있다?

지금까지 심장근육은 일생을 거쳐서 몇번 재생이 된다고 알려져 있습니다.

물론, 우리몸의 모든 세포들, 심지어 뇌세포까지도 몇번 완전히 재생이 된다고 합니다.

이 과정에서 줄기세포가 관여한다는 것은 당연한 이론이었지요.

그런데. 줄기세포없이, 이미 분화한 심장 근육세포가 다시 분열을 일으켜서 손상된 심장근육을
대체한다고 하면, 믿을 수 있을까요?

growth factor called neuregulin1 (NRG1)이라는 것을 주면 이것이 가능하다고 합니다.

심근경색으로 심근이 손상된 쥐의 완전 분화된 심장세포에 이 NRG1을 주입하였더니

이미 완전 분화한 심근세포가 분열을 일으켜서 손상된 심근을 대체할 새롭고 건강한 심장세포를 만들었다고 하네요.

이에 대한 보고가 계속 나오고 있습니다.

앞으로는 줄기세포를 주입하는 시술이 아니고 상황에 따라 적합한  Growth factors를 국소 주입하는
시술이 발달할 것 같습니다.

이 growth factor는 면역시스템을 피해가서 타인의 것을 쓸 수도 있을 것 같습니다.

마치 albumin을 혈관주사 할 수 있는 것처럼요.

Stem cells are cells found in most, if not all, multi-cellular organisms. They are characterized by the ability to renew themselves through mitotic cell division and differentiating into a diverse range of specialized cell types. Research in the stem cell field grew out of findings by Canadian scientists Ernest A. McCulloch and James E. Till in the 1960s.[1][2] The two broad types of mammalian stem cells are: embryonic stem cells that are isolated from the inner cell mass of blastocysts, and adult stem cells that are found in adult tissues. In a developing embryo, stem cells can differentiate into all of the specialized embryonic tissues. In adult organisms, stem cells and progenitor cells act as a repair system for the body, replenishing specialized cells, but also maintain the normal turnover of regenerative organs, such as blood, skin or intestinal tissues.

Stem cells can now be grown and transformed into specialized cells with characteristics consistent with cells of various tissues such as muscles or nerves through cell culture. Highly plastic adult stem cells from a variety of sources, including umbilical cord blood and bone marrow, are routinely used in medical therapies. Embryonic cell lines and autologous embryonic stem cells generated through therapeutic cloning have also been proposed as promising candidates for future therapies.

  • Self-renewal - the ability to go through numerous cycles of cell division while maintaining the undifferentiated state.
  • Potency - the capacity to differentiate into specialized cell types. In the strictest sense, this requires stem cells to be either totipotent or pluripotent - to be able to give rise to any mature cell type, although multipotent or unipotent progenitor cells are sometimes referred to as stem cells.

Potency definitions

Pluripotent, embryonic stem cells originate as inner mass cells within a blastocyst. The stem cells can become any tissue in the body, excluding a placenta. Only the morula's cells are totipotent, able to become all tissues and a placenta.

Potency specifies the differentiation potential (the potential to differentiate into different cell types) of the stem cell.[4]

  • Totipotent (a.k.a omnipotent) stem cells can differentiate into embryonic and extraembryonic cell types. Such cells can construct a complete, viable, organism.[4] These cells are produced from the fusion of an egg and sperm cell. Cells produced by the first few divisions of the fertilized egg are also totipotent.[citation needed]
  • Pluripotent stem cells are the descendants of totipotent cells and can differentiate into nearly all cells,[4] i.e. cells derived from any of the three germ layers.[5]
  • Multipotent stem cells can differentiate into a number of cells, but only those of a closely related family of cells.[4]
  • Oligopotent stem cells can differentiate into only a few cells, such as lymphoid or myeloid stem cells.[4]
  • Unipotent cells can produce only one cell type, their own,[4] but have the property of self-renewal which distinguishes them from non-stem cells (e.g. muscle stem cells).

Identification

The practical definition of a stem cell is the functional definition - a cell that has the potential to regenerate tissue over a lifetime. For example, the gold standard test for a bone marrow or hematopoietic stem cell (HSC) is the ability to transplant one cell and save an individual without HSCs. In this case, a stem cell must be able to produce new blood cells and immune cells over a long term, demonstrating potency. It should also be possible to isolate stem cells from the transplanted individual, which can themselves be transplanted into another individual without HSCs, demonstrating that the stem cell was able to self-renew.

Properties of stem cells can be illustrated in vitro, using methods such as clonogenic assays, where single cells are characterized by their ability to differentiate and self-renew.[6][7] As well, stem cells can be isolated based on a distinctive set of cell surface markers. However, in vitro culture conditions can alter the behavior of cells, making it unclear whether the cells will behave in a similar manner in vivo. Considerable debate exists whether some proposed adult cell populations are truly stem cells.

Embryonic

Embryonic stem cell lines (ES cell lines) are cultures of cells derived from the epiblast tissue of the inner cell mass (ICM) of a blastocyst or earlier morula stage embryos.[8] A blastocyst is an early stage embryo—approximately four to five days old in humans and consisting of 50–150 cells. ES cells are pluripotent and give rise during development to all derivatives of the three primary germ layers: ectoderm, endoderm and mesoderm. In other words, they can develop into each of the more than 200 cell types of the adult body when given sufficient and necessary stimulation for a specific cell type. They do not contribute to the extra-embryonic membranes or the placenta.

Nearly all research to date has taken place using mouse embryonic stem cells (mES) or human embryonic stem cells (hES). Both have the essential stem cell characteristics, yet they require very different environments in order to maintain an undifferentiated state. Mouse ES cells are grown on a layer of gelatin and require the presence of Leukemia Inhibitory Factor (LIF).[9] Human ES cells are grown on a feeder layer of mouse embryonic fibroblasts (MEFs) and require the presence of basic Fibroblast Growth Factor (bFGF or FGF-2).[10] Without optimal culture conditions or genetic manipulation,[11] embryonic stem cells will rapidly differentiate.

A human embryonic stem cell is also defined by the presence of several transcription factors and cell surface proteins. The transcription factors Oct-4, Nanog, and Sox2 form the core regulatory network that ensures the suppression of genes that lead to differentiation and the maintenance of pluripotency.[12] The cell surface antigens most commonly used to identify hES cells are the glycolipids SSEA3 and SSEA4 and the keratan sulfate antigens Tra-1-60 and Tra-1-81. The molecular definition of a stem cell includes many more proteins and continues to be a topic of research.[13]

After nearly ten years of research[14], there are no approved treatments using embryonic stem cells. The first human trial was approved by the US Food & Drug Administration in January 2009[15]. ES cells, being pluripotent cells, require specific signals for correct differentiation - if injected directly into another body, ES cells will differentiate into many different types of cells, causing a teratoma. Differentiating ES cells into usable cells while avoiding transplant rejection are just a few of the hurdles that embryonic stem cell researchers still face.[16] Many nations currently have moratoria on either ES cell research or the production of new ES cell lines. Because of their combined abilities of unlimited expansion and pluripotency, embryonic stem cells remain a theoretically potential source for regenerative medicine and tissue replacement after injury or disease.

Fetal

Fetal stem cells are primitive cell types found in the organs of fetuses [17]. The classification of fetal stem cells remains unclear and this type of stem cell is currently often grouped into an adult stem cell. However, a more clear distinction between the two cell types appears necessary.

The term adult stem cell refers to any cell which is found in a developed organism that has two properties: the ability to divide and create another cell like itself and also divide and create a cell more differentiated than itself. Also known as somatic (from Greek Σωματικóς, "of the body") stem cells and germline (giving rise to gametes) stem cells, they can be found in children, as well as adults.[18]

Pluripotent adult stem cells are rare and generally small in number but can be found in a number of tissues including umbilical cord blood.[19] A great deal of adult stem cell research has focused on clarifying their capacity to divide or self-renew indefinitely and their differentiation potential.[20] In mice, pluripotent stem cells are directly generated from adult fibroblast cultures. Unfortunately, many mice don't live long with stem cell organs.[21]

Most adult stem cells are lineage-restricted (multipotent) and are generally referred to by their tissue origin (mesenchymal stem cell, adipose-derived stem cell, endothelial stem cell, etc.).[22][23]

Adult stem cell treatments have been successfully used for many years to treat leukemia and related bone/blood cancers through bone marrow transplants.[24] Adult stem cells are also used in veterinary medicine to treat tendon and ligament injuries in horses.[25]

The use of adult stem cells in research and therapy is not as controversial as embryonic stem cells, because the production of adult stem cells does not require the destruction of an embryo. Additionally, because in some instances adult stem cells can be obtained from the intended recipient, (an autograft) the risk of rejection is essentially non-existent in these situations. Consequently, more US government funding is being provided for adult stem cell research.[26]

Amniotic

Multipotent stem cells are also found in amniotic fluid. These stem cells are very active, expand extensively without feeders and are not tumorogenic. Amniotic stem cells are multipotent and can differentiate in cells of adipogenic, osteogenic, myogenic, endothelial, hepatic and also neuronal lines.[27]

Lineage

To ensure self-renewal, stem cells undergo two types of cell division (see Stem cell division and differentiation diagram). Symmetric division gives rise to two identical daughter cells both endowed with stem cell properties. Asymmetric division, on the other hand, produces only one stem cell and a progenitor cell with limited self-renewal potential. Progenitors can go through several rounds of cell division before terminally differentiating into a mature cell. It is possible that the molecular distinction between symmetric and asymmetric divisions lies in differential segregation of cell membrane proteins (such as receptors) between the daughter cells.[28]

An alternative theory is that stem cells remain undifferentiated due to environmental cues in their particular niche. Stem cells differentiate when they leave that niche or no longer receive those signals. Studies in Drosophila germarium have identified the signals dpp and adherens junctions that prevent germarium stem cells from differentiating.[29][30]

The signals that lead to reprogramming of cells to an embryonic-like state are also being investigated. These signal pathways include several transcription factors including the oncogene c-Myc. Initial studies indicate that transformation of mice cells with a combination of these anti-differentiation signals can reverse differentiation and may allow adult cells to become pluripotent.[31] However, the need to transform these cells with an oncogene may prevent the use of this approach in therapy.

Treatments

Diseases and conditions where stem cell treatment is promising or emerging. [32] Bone marrow transplantation is, as of 2009, the only established use of stem cells.

Medical researchers believe that stem cell therapy has the potential to dramatically change the treatment of human disease. A number of adult stem cell therapies already exist, particularly bone marrow transplants that are used to treat leukemia.[33] In the future, medical researchers anticipate being able to use technologies derived from stem cell research to treat a wider variety of diseases including cancer, Parkinson's disease, spinal cord injuries, Amyotrophic lateral sclerosis, multiple sclerosis, and muscle damage, amongst a number of other impairments and conditions.[34][35] However, there still exists a great deal of social and scientific uncertainty surrounding stem cell research, which could possibly be overcome through public debate and future research, and further education of the public.

Stem cells, however, are already used extensively in research, and some scientists do not see cell therapy as the first goal of the research, but see the investigation of stem cells as a goal worthy in itself.[36]
